Siuslaw Open at Florence Golf Links 7:00 AM to 7:00 PM About The Tournament
This year’s tournament is being held at
beautiful Florence Golf Links (formerly
Sandpines) in Florence, Oregon.
Registration is limited to 100 golfers for each
pool. We will be following COVID
Guidelines by staggering tee times. The
tournament is a 4-person team, “ Calloway
Scoring” format. A putting contest will also
be held between 7:00 am and 3:45 pm. A
delicious meal will be provided during the
tournament. Individual and team prizes will
be awarded post-tournament. Distancing
regulations will be adhered to. Carts will be
sanitized for your protection.

Registration no later than May 14th
Team Registration before May 1st: $360
Team Registration after May 1st: $380
(Team Fee includes: 3 putts for each player in the
putting contest, 3 mulligans for each player, meal,
snacks, and beverages)

Payment can be made by check or credit card. For
mail-in registrations use the form included in this
brochure and make a check payable to Siuslaw
Outreach Services. Registrations can also be done

Saturday: Tee times begin at 8:00 am – Six Pack
Sunday: Tee times begin at 8:00 am - Chapman
CHECK-IN FOR REGISTERED PLAYERS

USGA handicap maximum is 36 for Men & 40 for Ladies
with a handicap revision date of May 21, 2021
Divisions: Men’s White/Ladies Red; Men’s Gold/Ladies Red
All players must have posted a minimum of 5 Rounds of play in
the previous year. Men electing to play from the Gold tees
must have posted at least 50% of scores from the Gold tees.
